We present a 5-year performance of residents on the ISE exam who subsequently were administered the QE to better define if earlier time points in training may identify trainees who would benefit from additional educational support. METHODS: A shared data use agreement between the ABU and the AUA permitted review of ISE scores over a 5-year period from 2014-2018 and QE scores in 2019. Those with a QE score in 2019 but no ISE or missing 1 or more ISE scores were excluding, yielding a final cohort of 284. Pearson's correlation coefficient measured the association between percentage correct for each ISE year and the QE. The Youden Index calculated the optimal cut-point for yearly ISE percentage correct that would predict scoring greater than the lowest quartile and decile on the QE. RESULTS: Median percent questions correct increased between post graduate year (PGY) 1 (47%) and PGY2 (56.5%) years but remained stable thereafter (PGY3 – 5) at approximately 70%. Median QE percent correct in 2019 was 66.2% (Std Dev 7.55%). The correlation of percent questions correct between ISE and QE improved from 0.31 to 0.53 during the course of training (Table 1). The lowest decile and quartile percent correct scores on the QE were 56% and 60%, respectively. Table 2 summarizes the probability of scoring above the lowest decile or quartile on the QE as stratified by PGY year. Percent correct ISE score predicting performance above the lowest decile 2019 QE score increased from 38% PGY1, 57% PGY2, and leveled off after PGY3 year (∼ 70%). Similar observations were noted with lowest quartile QE score. CONCLUSIONS: Scoring 70% on the ISE during PGY3 and later was associated with low risk of failing the QE. We identify percent correct on ISE scores at early PGY associates with poorer performance on the QE. Such information provides benchmarks for residency programs to offer targeted educational content for at-risk candidates.
