1Effects of Increasing the Concentration of Dialysate Magnesium on Cardiovascular Health: A Narrative Review
Bryn Tannar, Hareth Al-Hellawi,Jessica M Sontrop, Rey R Acedillo, Ahmed A Al-Jaishi, Sierra Anderson, Amit Bagga, David Berry, Eliot Beaubien, Peter G Blake,Clara Bohm,Pierre Antoine Brown,

Purpose of review: Cardiovascular disease (CVD) accounts for nearly half of deaths among people receiving maintenance hemodialysis. Observational and interventional data suggest that higher serum magnesium, achieved through higher dialysate magnesium concentrations or oral supplementation, may improve cardiovascular outcomes and survival. This review synthesizes current evidence and provides context for an ongoing cluster-randomized trial that is testing whether a center-wide dialysate magnesium concentration of 0.75 mmol/L, versus ≤0.50 mmol/L, delivered as a policy and sustained for up to four years, reduces the risk of major cardiovascular-related hospitalizations. Sources of information: Peer-reviewed articles. Methods: We searched MEDLINE and EMBASE for observational and interventional studies evaluating serum or dialysate magnesium concentrations and cardiovascular outcomes in patients with chronic kidney disease and/or kidney failure. We appraised the methodological quality of interventional trials. This review is divided into four sections: (1) epidemiological associations between serum magnesium concentrations and CVD outcomes, (2) the impact of a higher concentration of dialysate magnesium on CVD outcomes, (3) the impact of oral magnesium supplementation on CVD outcomes, and (4) ongoing trials. Key findings: Twenty studies, including 10 randomized controlled trials, were reviewed. Systematic reviews and meta-analyses show that hypomagnesemia is associated with higher risks of cardiovascular events and all-cause mortality in hemodialysis. Interventional studies indicate that higher dialysate magnesium concentrations or oral supplementation can improve surrogate markers of vascular health, including less vascular calcification and stiffness. Higher dialysate concentrations may also lower cardiovascular mortality. Given encouraging but predominantly surrogate-based evidence, adequately powered randomized trials are warranted. Limitations: Most trials were small, single-center, and of short duration. They relied on surrogate endpoints, and there was heterogeneity in interventions and outcome measures.

2A Multipronged Intervention to Improve Outpatient Care Following Hospitalization with Acute Kidney Injury: an Implementation Study Protocol
Michelle M Y Wong, Peter Birks, Omosomi Enilama, Janelle S W Yu, Anurag Singh,Adeera Levin

Background: Acute kidney injury (AKI)-related hospitalizations are associated with long-term health consequences, but post-AKI management and monitoring in the outpatient primary care setting are suboptimal. Documentation and communication about in-hospital AKI episodes to patients’ primary care providers are often inadequate. Awareness of AKI among patients is generally poor. Objective: To develop a multifaceted intervention package to improve post-AKI care, to evaluate its implementation using feasibility and process metrics, and to observe its short-term effectiveness. Design: Mixed-methods feasibility study with type 3 hybrid implementation/effectiveness design. Participants: Kidney specialist physicians on inpatient consultation service at three hospital sites in British Columbia (in New Westminster, Prince George, Vancouver) will deliver the intervention package to patients with AKI and their primary care providers. Measurements: Implementation outcomes include reach, adoption, fidelity, and sustainment of the intervention. Implementation determinants, including acceptability, adaptability, feasibility, appropriateness, and satisfaction, will be assessed with surveys and focus groups with kidney specialist physicians, primary care providers, and patients/caregivers. Short-term effectiveness outcomes include: patient follow-up visits with primary care providers, and kidney lab testing at three months; AKI awareness and self-efficacy among primary care providers and patients. Methods: Guided by the Theoretical Domains Framework, we conducted focus groups with interest holders to identify the barriers, challenges, and needs in post-AKI care and then applied this information to develop an intervention package, including an AKI-specific discharge/sign-off template, a post-AKI medication management guide, educational websites for providers and patients, and a patient handout. In this six-month implementation feasibility study, the implementation strategies will be initiated sequentially at each site for four to eight weeks. Quantitative data will be collected via self-report, chart review, surveys, and website hits and analyzed using descriptive statistics. Postimplementation qualitative data will be collected from focus groups/interviews and will be analyzed by identifying codes and categorizing them into themes. Limitations: The study will take place in the setting of nephrology consultation services. Adaptations to implementation strategies would be required for delivery of the intervention by other inpatient providers. Conclusions: The study will evaluate feasibility and preliminary effectiveness of a multifaceted intervention aimed at improving care coordination between specialists and primary care providers and care transition following hospital discharge in patients experiencing AKI. Study Registration: This study is not registered.

3Improving Sonographic Visualisation of the Appendix in a Regional Referral Hospital
Shyr Chui, Carly Phinney, Karina Hansen, Deanna Danskin

Ultrasound is a first-line and often preferred imaging modality in the diagnosis of acute appendicitis. When the appendix is not visualised during a dedicated appendix ultrasound study, patients may require a CT study, which uses ionising radiation, or undergo conservative clinical observation with the inherent risk of clinical deterioration, perforation and sepsis. Median baseline data, at our hospital imaging department, revealed a rate of combined normal and abnormal appendix visualisation of 34.5% which is below the reported visualisation rates in the North American literature and well below the rates reported in the global literature. We embarked on a formal quality improvement (QI) project to improve the rates of appendix visualisation in our hospital ultrasound department. Using the Model of Improvement framework and a team approach, we generated and trialled multiple plan-do-study-act interventions over a project term of 12 months. In the second half of the project term, we saw a sustained rise in appendix visualisation exceeding our original stretch goal of 75% visualisation which was sustained 6 months after the formal project end (p<0.001). This rise was accompanied by a commensurate increase in sonographer confidence in appendix visualisation. In our case, the Model of Improvement methodology proved successful in solving our complex problem of sonographic appendix under-visualisation. The learnings of this QI project have been widely shared and spread according to the ethos of QI.

4Pathologist Workload, Burnout, and Wellness: Connecting the Dots
Ziyad Khatab, Kattreen Hanna, Andrew Rofaeil,Catherine Wang,Raymond Maung, George M. Yousef

No standard tool to measure pathologist workload currently exists. An accurate measure of workload is needed for determining the number of pathologists to be hired, distributing the workload fairly among pathologists, and assessing the overall cost of pathology consults. Initially, simple tools such as counting cases or slides were used to give an estimate of the workload. More recently, multiple workload models, including relative value units (RVUs), the Royal College of Pathologists (RCP) point system, Level 4 Equivalent (L4E), Work2Quality (W2Q), and the University of Washington, Seattle (UW) slide count method, have been developed. There is no "ideal" model that is universally accepted. The main differences among the models come from the weights assigned to different specimen types, differential calculations for organs, and the capture of additional tasks needed for safe and timely patient care. Academic centers tend to see more complex cases that require extensive sampling and additional testing, while community-based and private laboratories deal more with biopsies. Additionally, some systems do not account for teaching, participation in multidisciplinary rounds, quality assurance activities, and medical oversight. A successful workload model needs to be continually updated to reflect the current state of practice.Awareness about physician burnout has gained attention in recent years and has been added to the World Health Organization's International Classification of Diseases (World Health Organization, WHO) as an occupational phenomenon. However, the extent to which this affects pathologists is not well understood. According to the WHO, burnout syndrome is diagnosed by the presence of three components: emotional exhaustion, depersonalization from one's work (cynicism related to one's job), and a low sense of personal achievement or accomplishment. Three drivers of burnout are the demand for productivity, lack of recognition, and electronic health records. Prominent consequences of physician burnout are economic and personal costs to the public and to the providers.Wellness is physical and mental well-being that allows individuals to manage stress effectively and to thrive in both their professional and personal lives. To achieve wellness, it is necessary to understand the root causes of burnout, including over-work and working under stressful conditions. Wellness is more than the absence of stress or burnout, and the responsibility of wellness should be shared by pathologists themselves, their healthcare organization, and governing bodies. Each pathologist needs to take their own path to achieve wellness.

5Prevalence of Coronary Atherosclerosis in Master Female Endurance Athletes
    Efstathios Papatheodorou,Vincent L Aengevaeren,Thijs M H Eijsvogels,Khaled AlFakih,Rebecca Kathryn Hughes,Ahmed Merghani,Christine K Kissel,Saad Fyyaz,Athanasios Bakalakos,Mathew G Wilson,Damini Dey,Gherardo Finocchiaro,

ABSTRACT BACKGROUND Studies in ostensibly healthy male master athletes have revealed a greater prevalence of coronary artery calcification (CAC) and coronary plaques compared with relatively sedentary counterparts. In contrast, data relating to potentially adverse coronary remodelling in female master athletes is sparse and conflicting. We investigated the prevalence of coronary atherosclerosis in a cohort of predominantly post-menopausal female master athletes with a low atherosclerotic risk profile. METHODS 196 female athletes with a mean age of 55±7 years-old and median exercise duration of 33 years (25-39) and 59 relatively sedentary females of similar age underwent cardiovascular investigations including a coronary computed tomogram angiography with assessment of CAC scores, coronary plaques, and pericoronary adipose tissue attenuation. 70% of the athletes and 68% of the control group were post-menopausal. RESULTS Athletes and controls had a similarly low Framingham 10-year risk (1.49% versus 2.1%; P=0.68), but body mass index and blood pressure were lower and HDL-C was higher in the athletes. The prevalence of CAC score >0 Agatston units (AU) was low and did not differ between athletes and controls (21% versus 32%;P=0.073). Female athletes had a lower prevalence of a CAC score >50 th centile (19% versus 32%;P<0.03) and >75 th centile (14% versus 25%;P=0.045) for age compared with controls, but the prevalence of a CAC score >100 AU did not differ between the groups (3.6% versus 8.5%;P=0.12). There were also no differences between the groups in the prevalence of individuals with coronary plaques (21% versus 32%;P=0.09), total plaque volume (16 mm 3 [IQR 3-56] versus 49 [5-142] mm 3 ;P=0.08), or plaque burden (10.8% [2.8-21] versus 15.4% [4.6-28];P=0.46). Coronary plaques were predominantly calcified in both athletes and controls (80% versus 63%;P=0.08). Age, blood pressure and HDL-C were independent predictors for a CAC score >0 AU among athletes. CONCLUSIONS In contrast with previous studies in male master athletes, lifelong exercise in female counterparts does not appear to be associated with increased CAC score, coronary plaque burden or any qualitative differences in coronary plaque compared with relatively sedentary healthy counterparts. Coronary atherosclerosis in master female athletes is mainly driven by traditional risk factors.
