Athlete fatigue and overtraining are critical factors affecting performance and health, yet traditional evaluation methods relying on subjective judgment or single-indicator monitoring lack systematic and real-time capability. This study proposes a novel Meta-Learning Ensemble Framework (MLEF) integrating multidimensional physiological monitoring for intelligent fatigue risk prediction. The MLEF architecture consists of three progressive layers: a Feature Selection Layer using ANOVA F-statistic based univariate selection to identify the top 12 features from 15 original variables, a Base Learner Layer training four heterogeneous logistic regression classifiers with different regularization configurations, and a Meta-Learning Layer integrating predictions through weighted voting and stacking ensemble strategies. We constructed experiments on the AFR-1000 dataset containing 1000 athletes with balanced class distribution (51:49 normal/fatigue), split 8:2 into training and testing sets with stratified sampling. On the independent test set, MLEF achieved 99.00
Introduction Delayed or obstructed labor closely associated with adverse pregnant outcomes, yet modifiable physiological factors influencing labor duration remain unclear. Electrolytes play a crucial role in maintaining uterine contractility and physiological balance during labor. However, existing evidence on their relationship with labor duration is limited, inconsistent, and based on small or homogeneous populations. To investigate the associations between maternal serum electrolyte levels and the duration of the first and second stages of labor. Methods General linear regression showed that higher K + levels were associated with both prolonged first-stage and second-stage labor, while Na + had opposing effects. Cl − reduced second-stage duration, whereas Mg 2+ prolonged it. Poisson regression revealed that Na + decreased first-stage prolongation riskbut increased second-stage risk. Ca 2+ strongly protected against prolonged second-stage labor, while Mg 2+ reduced first-stage risk. While decreased serum Ca 2+ content is closely associated with the prolonged second stage of labor (per unit increase: RR: 0.251; 95% CI: 0.132, 0.475). GAM showed the nonlinear relationships of five serum electrolytes with both labor duration. Furthermore, five electrolytes were positively correlated with the first and second labor duration, evidenced by Q-g model. Discussion These findings suggest that electrolyte imbalances may influence labor progression through their effects on uterine contractility and neuromuscular function.Maternal serum electrolyte levels are significantly associated with labor duration. Monitoring and managing electrolyte balance may provide a practical strategy to optimize labor outcomes and reduce the risk of prolonged labor.
Background: Hereditary spherocytosis (HS) is a prevalent form of hereditary hemolytic anemia, primarily attributed to defects in the membrane skeleton proteins of red blood cells. The SPTB gene is one of the principal genes implicated in the pathogenesis of this condition. Nevertheless, the pathogenic mechanisms associated with numerous novel SPTB mutations remain inadequately understood, thereby hindering advancements in precise diagnostic and therapeutic strategies. Methodology: A novel heterozygous SPTB mutation (c.544T>C, p.Trp182Arg) was identified in a Chinese patient with hereditary spherocytosis (HS) and subsequently validated via Sanger sequencing. The evolutionary conservation and potential functional implications of this mutation were assessed through cross-species sequence alignment and splicing prediction analyses. Protein conformational changes resulting from the mutation were examined using protein structure modeling. Stable cell lines were established utilizing the PiggyBac transposon system, and gene and protein expression levels were quantified through quantitative reverse transcription PCR (qRT-PCR) and Western blotting, respectively. Additionally, transcriptomic and proteomic analyses were conducted to investigate the mutation's impact on downstream molecular pathways. Flow cytometry was employed to analyze cell cycle distribution and evaluate the functional consequences of the mutation. Results: Cross-species sequence alignment revealed that the Trp182 residue is highly conserved. Structural modeling indicated that the Trp to Arg substitution alters the local side chain conformation. Functional assays have demonstrated a significant reduction in the expression levels of SPTB mRNA and protein within cells harboring the mutation. Multi-omics analyses have elucidated that this mutation disrupts several critical pathways, including those involved in cell senescence, AMPK signaling, and DNA repair, with the differentially expressed proteins predominantly localized in the nucleus and cytoplasm. Flow cytometry analysis indicated an increased proportion of mutant cells in the G0/G1 phase and a decreased proportion in the G2/M phase, suggesting a compromised proliferative capacity. Additionally, an elevated positive rate of SA-β-gal staining corroborated the emergence of a senescent cell phenotype. Conclusion : This study provides the first evidence confirming that the SPTB p.Trp182Arg variant is a novel pathogenic mutation responsible for the onset of hereditary spherocytosis (HS). Mechanistically, this mutation compromises protein stability, disrupts essential cellular signaling pathways, and induces cellular senescence. These findings not only broaden the mutational spectrum of the SPTB gene and offer a theoretical foundation for clinical genetic counseling but also reveal the potential role of cellular senescence in the pathological progression of HS. This insight presents a new research avenue for developing targeted therapeutic strategies.
INTRODUCTION: Bariatric case managers (BCMs) are crucial in serving clients undergoing bariatric surgery in China. Understanding the perspectives of experienced BCMs regarding their roles in bariatric case management remains limited. This study seeks to understand the perceptions of experienced BCMs regarding their roles in bariatric care. METHODS: This qualitative study utilized semi-structured interviews with 29 experienced BCMs, each having more than three years of involvement in the field or experience with over 1000 patients. Interviews were audio-recorded, transcribed verbatim, and thematically analyzed. RESULTS: Three themes emerged regarding BCM perceptions: “The ‘Prevention-Intervention-Feedback’ Closed-Loop System in Comprehensive Safety Management”, “Dual Tension between Emotional Labor and Patient Empowerment”, and “Professional Identity Reconstruction within Systemic Constraints”. Experienced BCMs expressed mixed feelings about their roles, emphasizing safety for patient management. They also advocated standards for BCMs. CONCLUSION: The findings highlight the unique role and contributions of the BCM within the Chinese context. Experienced BCMs prioritize patient safety while providing case management. Developing a national BCM alliance or fostering bariatric nurse specialists who establish standards for bariatric case management is essential for future professionalization. A national care and research platform is advocated for quality improvement.
