O artigo analisa experiências formativas em saúde por meio de metodologia Balint-Paideia, articulando Saúde Coletiva e teoria psicanalítica, especialmente o conceito de inconsciente capitalista de Tomšič. Com base em observação participante e diários de campo de cursos de especialização e extensão em Saúde da Família realizados entre 2018 e 2020, investigou-se como trabalhadores elaboram tensões entre desejo de transformar práticas e limites impostos pela lógica capitalista da produção do cuidado. Os resultados mostram que, embora marcados pela alienação e pela captura do desejo pelo imperativo produtivo, os grupos funcionaram como espaços protegidos de palavra, reconhecimento e afeto, possibilitando a nomeação de questões subjetivas, o reposicionamento diante do sofrimento e a invenção coletiva de práticas. Conclui-se que a formação, ao integrar clínica, política e subjetividade, pode abrir brechas emancipatórias, ampliando o horizonte da Reforma Sanitária e recolocando o sujeito no centro da produção do cuidado.
The article analyzes training experiences in health through the Balint-Paideia methodology, articulating Collective Health and psychoanalytic theory, especially Tom & scaron;i & ccaron;'s concept of the capitalist unconscious. Based on participant observation and field diaries from specialization and extension courses in Family Health held between 2018 and 2020, the study investigated how workers deal with the tensions between the desire to transform practices and the limits imposed by the capitalist logic of care production. Results show that, although marked by alienation and by the capture of desire through the productive imperative, the groups functioned as protected spaces of speech, recognition, and affection, enabling the naming of subjective issues, repositioning in the face of suffering, and the collective invention of practices. It concludes that training, by integrating clinic, politics, and subjectivity, can open emancipatory breaches, broadening the horizon of the Brazilian Health Reform and restoring the subject to the center of care production.
O estudo objetiva analisar comunicações visuais dispostas na recepção de Unidades Básicas de Saúde (UBS). Trata-se de um estudo qualitativo, realizado em 16 UBS do município de Içara, estado de Santa Catarina, Brasil. Foi realizado um registro fotográfico, conforme análise semiótica, que envolveu escolha das imagens, inventário denotativo, estabelecimento de níveis de significação, decisão de cessação e relatório. Para organização dos dados, foi utilizado o software Atlas.ti 22. Os resultados indicaram cinco categorias: materiais de apoio ao processo de trabalho em saúde (38,5%), informações sobre as UBS (26,7%), dificuldades estruturais das UBS (15,7%), recomendações da equipe de saúde aos usuários (15,7%) e objetos decorativos das recepções em UBS (3,4%). As UBS devem possuir suas recepções com poucos materiais de apoio e informes; o mobiliário precisa estar em condições adequadas; e cores muito fortes, fios elétricos expostos, pisos danificados e simbologia religiosa devem ser evitadas.
This article deals with the interest of the scientific field in systematizing the co-management investigative praxis, in a health emergency scenario, based on the analysis of a research with a multicentric qualitative approach, using the framework of the Support Research and the analysis of critical hermeneutics. As a result, it was identified that the creation of a map guide contributed as a guiding document, aiming at organizing different techniques for the organization and formation of field researchers, as well as an instrument of data analysis. The training of researchers for the theoretical framework of Support Research, as well as their co-management and involvement in the different stages of research, proved to be a differential for the production of subjects and collectives with investigative praxis, allowing a dialogic exchange between coordinators and researchers and regular sharing of the results. It is concluded that the way in which the methodology was proposed, allowed the expansion of the reflective capacity and understanding of reality, contributing to the formation of researchers as active and critical subjects in the process of data collection, analysis and discussion, encouraging sensitive and attentive actions while seeking to identify the particularities of each context.
Resumo Este ensaio desenvolve argumentos e propostas a fim de fornecer elementos para a construção e implementação de uma nova política e gestão de pessoal para o Sistema Único de Saúde (SUS). Trata-se de um trabalho voltado tanto para auxiliar pesquisadores e especialistas em saúde coletiva, quanto para apoiar movimentos sociais criados para assegurar a sustentabilidade do SUS e condições adequadas de trabalho aos profissionais.
The aim of this study was to analyze visual communication media displayed in the reception areas of Primary Care Centers (PCCs). We conducted a qualitative study of 16 PCCs in I & ccedil;ara, Santa Catarina, Brazil. We took photographs of the media and conducted a semiotic analysis involving the following stages: selection of images, denotative inventory, analysis of levels of meaning, cessation decision and reporting. The data were organized using ATLAS.ti 22. The results indicated five categories of media: health care support materials (38.5%); information on PCCs (26.7%); PCC structural problems (15.7%); health team patient recommendations (15.7%); and decorative objects (3.4%). PCCs should display few support materials and notices in the reception area; furniture needs to be in good condition; bright colors, exposed electric cables, damaged floors and religious symbols should be avoided.
Resumo O artigo trata do interesse do campo científico em sistematizar a práxis investigativa cogestora em cenário de emergência sanitária a partir da análise de uma pesquisa de abordagem qualitativa multicêntrica, valendo-se do referencial da pesquisa-apoio e da análise da hermenêutica critica. Como resultados, identificou-se que a elaboração de um guia-mapa contribuiu como documento norteador, com o objetivo de organizar diferentes técnicas para a preparação e formação dos pesquisadores de campo, também como instrumento de análise dos dados. A formação de pesquisadores para o referencial teórico da pesquisa-apoio, assim como a cogestão e a implicação deles nas diferentes etapas da pesquisa, mostrou-se como diferencial para produção de sujeitos e coletivos com a práxis investigativa, permitindo a troca dialógica dentre coordenadores e pesquisadores e o compartilhamento regular dos resultados. Conclui-se que a forma como a metodologia foi proposta possibilitou a ampliação da capacidade reflexiva e de compreensão sobre a realidade, contribuindo para a formação de pesquisadores como sujeitos ativos e críticos no processo de coleta, análise e discussão dos dados, incentivando a atuação sensível e atenta ao mesmo tempo em que buscou identificar as particularidades de cada contexto.
Resumo A pandemia de COVID-19 no Brasil atingiu níveis alarmantes. Na cidade do Rio de Janeiro, ela encontrou um cenário de desmonte da Atenção Primária à Saúde (APS) em meio à uma crise política, o que teve grande impacto nos territórios de maior vulnerabilidade. O objetivo desse estudo é analisar de que forma as favelas e equipes da APS organizaram-se para desenvolver ações comunitárias, ocupando espaços deixados pela falta de outras políticas públicas. Os resultados fazem parte da pesquisa qualitativa multicêntrica “Estratégias de abordagem dos aspectos subjetivos e sociais na Atenção Primária no contexto da pandemia”, onde foram analisados documentos orientadores públicos e 36 entrevistas em profundidade com trabalhadores e usuários da APS, organizadas em grades interpretativas. Como resultado, observou-se que houve iniciativas cogestoras dos trabalhadores e usuários da APS, a partir do surgimento de coletivos organizados e ativismo social, para o enfrentamento da pandemia, independente das normativas da Secretaria Municipal de Saúde e demais instâncias governamentais. A APS se apresentou como único equipamento público nos territórios de alta vulnerabilidade, onde a violência armada esteve presente mesmo durante a pandemia.
Ensaio sobre história da Saúde Coletiva como campo científico e político. Análise das características das três áreas da Saúde Coletiva: Epidemiologia; Ciências Sociais e Humanas; e Política, Planejamento e Gestão, mediante estudo temático de Congressos da Associação Brasileira de Saúde Coletiva (Abrasco). Constatam-se conflitos de interesse e de paradigmas entre as áreas, mas também estratégias de governança para sustentar a unidade do campo da Saúde Coletiva que poderia ser sintetizada com a expressão: dialética da autonomia e integração.
Resumo A pandemia de COVID-19 encontrou no Brasil um campo propício para sua propagação. A falta de coordenação de ações do governo federal, em conjunto com uma Atenção Primária à Saúde (APS) já fragilizada trouxe consequências para trabalhadores e usuários. O objetivo é analisar a gestão e o processo de trabalho na APS no enfrentamento à pandemia da COVID-19 em 3 municípios, considerando o processo histórico de sua constituição, a relação com o SUS e a APS e o ativismo social em territórios vulneráveis, a partir da percepção de usuários e trabalhadores. Foi realizada busca bibliográfica, observação participante e 97 entrevistas em profundidade com trabalhadores e usuários da APS. Os resultados foram organizados em 4 eixos: Implantação e gestão da APS nos municípios; Processo de trabalho na APS; Percepção dos trabalhadores e usuários em relação à APS e o SUS; Territórios, coletivos organizados e ativismo social. Os trabalhadores enfrentaram um ambiente de medo, sobrecarga e mudanças no processo de trabalho, o que não os impediu de resistir e reinventar suas práticas. Os usuários conviveram com barreiras de acesso e dificuldades no cumprimento de medidas sanitárias, mas foram encontradas experiências de coletivos organizados que surgiram como forma de enfrentamento à pandemia.
This essay discusses the history of public health as a scientific and political field. We analyze the main features of the three areas of public health (epidemiology, social and human sciences, and policy, planning and management) based on a thematic study of congresses held by the Brazilian Public Health Association (Abrasco). The findings highlight a number of conflicting interests and paradigms among the areas, but also governance strategies to maintain the unity of field of public health that could be summed up with the phrase "dialectics of autonomy and integration".
Global health crisis: research, learning and building an institutional culture to constantly strengthen PHCConsidering the psychoanalytic premises, the possibility of working through grief or trauma 1 requires the naming and reappraisal of what was lost.It derives from the possibility of reflecting, problematizing, de-romanticizing and pondering what has been lost.Silence, on the other hand, does not help.The same reasoning can be applied to the COVID-19 pandemic, which has left traces -and grief -, that calls for discussion.In this issue, we have chosen Primary Health Care (PHC) to analyze the relationship between the Unified Health System (Sistema Único de Saúde -SUS) and the COVID-19 pandemic, due to its privileged interface with the population in the territories and with the other points of the care network 2 .During the COVID-19 pandemic, radical modifications occurred in work processes in PHC, with the interruption of routine procedures, the incorporation of the continuous use of Personal Protective Equipment (PPE) and the implementation of erratic and frequently changing protocols.The work overload was also notable when the coronavirus vaccines were made available.Undoubtedly, there are many elements that constitute raw material for research aimed at understanding the limitations and possibilities for advancement in expanded care in PHC.The articles published in this issue of Ciência & Saúde Coletiva deal with these issues, covering the perspectives of managers, workers and users.In particular, it presents results from two studies: "Primary Health Care policy in the context of the pandemic in the municipalities of São Paulo" and "Strategies for addressing subjective and social aspects in Primary Health Care in the context of the pandemic".The first, carried out by the Health Institute (SES-SP), in partnership with COSEMS-SP and the Primary Care technical area of the São Paulo State Health Department, was dedicated to producing information on the model of care and organization of PHC in the State of São Paulo, with an emphasis on discussion about continuity of care during the pandemic based on a survey with municipal managers and six case studies.The second study was developed by means of a partnership between Abrasco, Unicamp, the São Paulo Health Institute and Fiocruz-RJ and sought to analyze the perspectives of PHC users and workers regarding the subjective and organizational repercussions produced by its discontinuity during the first year of the pandemic.It used the methodology of in-depth interviews, with the elaboration of narratives.The fieldwork was carried out in areas of high social vulnerability in the municipalities of Campinas, São Paulo and Rio de Janeiro.Complementing the analytical efforts of these two studies are articles from other regions of Brazil, with equally relevant contributions.As a result of this compilation, this thematic issue features four initial articles that deal with structural and comprehensive issues, including mapping scientific production in PHC during the pandemic, a discussion on financial transfers, an analysis of the organization of services according to Brazilian regions and the relationship between the different ways in which PHC works, and the history of heterogeneity in the implementation of the Family Health Strategy model.There follow two papers that delve deeper into the reality of the state and the municipality of São Paulo.With five articles dedicated to the topic, a significant portion of this thematic issue discusses the functioning of arrangements, measures and actions aimed at operating in conjunction with the territory and community care, in line with the World Health Organization (WHO) recommendations 3 .There are also two articles reflecting on the mischaracterization of PHC and on the discretion of professionals as an important analytical component.This is a set of articles aimed at fostering debates and perspectives for enhancing the National Primary Health Care Policy, which is a key aspect for the advancement of the SUS.
The COVID-19 pandemic found fa-vourable conditions to spread in Brazil. A lack of coordinated action by the federal government, together with already weak Primary Health Care (PHC) had consequences for workers and users. This study examined PHC management and work process in response the COVID-19 pandem-ic in three municipalities, in view of the history of how PHC was set up, its relationship with the national public health system (SUS), and PHC and social activism in vulnerable territories, from the perceptions of users and workers. The study included a literature search, participant observa-tion and 97 in-depth interviews of PHC workers and users. The results were organised into four areas: PHC implementation and management in the municipalities; the PHC work process; work-ers' and users' perceptions of PHC and the SUS; and territories, organised groups and social activ-ism. Workers faced an environment of fear, over-work and changes in their work process, none of which prevented them from persisting and rein-venting their practice. Users encountered barriers to access and difficulties in complying with public health measures, but the study found experienc-es of organised groups that emerged as means of coping with the pandemic.
RESUMO Este artigo teve como objetivo analisar o trabalho multiprofissional na perspectiva interdisciplinar na formação de estudantes de medicina por meio de grupos Balint-Paideia. Trata-se de uma pesquisa qualitativa, descritiva, exploratória e analítica, realizada com graduandos do internato de saúde coletiva de uma universidade pública brasileira. Os internos participaram de grupos Balint-Paideia durante sua imersão nas equipes de saúde da família. Os dados foram coletados por meio de grupos focais e analisados mediante construção de narrativas. Foram encontrados os seguintes núcleos argumentativos: potencialidades e os entraves para o trabalho em equipe; influência da formação baseada no trabalho para o exercício da atividade colaborativa; papel dos grupos Balint-Paideia no processo formativo e na aprendizagem de conceitos relevantes para a Atenção Primária à Saúde. A estratégia pedagógica combinando prática na Atenção Primária à Saúde, vivência interprofissional e grupos Balint-Paideia, com elaboração de projeto terapêutico singular, apesar de desafiante, tende a propiciar um olhar que transcende a clínica estritamente biomédica e as relações assimétricas, evocando os estudantes de medicina à construção de uma práxis interdisciplinar.
This essay develops arguments and proposals to provide elements to construct and implement a new workforce policy and management for the Brazilian National Health System (SUS). This is awork focused both in helping collective health researchers and specialists and in supporting social movements created to assure the sustainability of the SUS and the adequate working conditions to professionals. The COVID-19 pandemic has reached alarming levels in Brazil. In Rio de Janeiro city, it arrived in a scenario in which Primary Health Care (PHC) was being dismantled in the midst of a political crisis, which had major impact on the most vulnerable territories. This study examined how favelas and PHC teams organised community-based action and occupy the vacuum left by the lack of public policies. The results form part of the multi-centre qualitative study “Strategies for approaching subjective and social aspects of Primary Care in the pandemic context”, using public guidance documents and 36 in-depth interviews of PHC workers and users, which were categorised into interpretive grids. Co-management initiatives by PHC workers and users were found to have arisen out of organised groups and social activism, to face the pandemic, independently of regulations from the Municipal Health Department and other government bodies. PHC figured as the only public facility in highly vulnerable territories, where armed violence was ongoing even during the pandemic.
Resumo Este artigo analisa as ofertas de cuidado da APS na cidade de Campinas-SP, durante a pandemia de COVID-19, tendo as mudanças no processo de trabalho como um guia condutor. Tratou-se de uma pesquisa descritiva de caráter qualitativo, incluindo observação participante e entrevistas em profundidade com trabalhadores e usuários de quatro Unidades Básicas de Saúde (de junho/2021 a janeiro/2022). As análises apontam uma diferença significativa entre o primeiro e o segundo ano da pandemia. No primeiro momento, houve desorganização das estratégias de cuidado e restrição do atendimento aos casos de COVID-19. Já no segundo ano, foram retomadas atividades como visitas domiciliares e atendimentos de rotina, que, somadas ao agravamento do contexto sociossanitário e às novas demandas, como a vacinação, provocaram sobrecarga de trabalho e acirramento das relações entre profissionais e usuários. Além disso, constatou-se a fragilização dos espaços coletivos e de cogestão, tanto no nível da gestão municipal, quanto em reuniões de equipe e espaços de participação social. A retomada de tais espaços e o cuidado aos trabalhadores desgastados por esse período constituem-se desafios para a Atenção Primária em Saúde no contexto pós-pandemia.
Abstract This article examines supply of Primary Health Care in the city of Campinas (São Paulo state) during the COVID-19 pandemic, taking changes in the work process as its guide. This descriptive, qualitative study included participant observation and in-depth interviews of workers and users at four PHC facilities, from June 2021 to January 2022. The analyses found significant differences between the first and second years of the pandemic. At first, care strategies were disorganised and care for COVID-19 cases, limited. In the second year, home visits and routine care were resumed. This, added to the worsening social and public health context and new demands, such as vaccination, caused overwork and strained relations between health personnel and users. Also, collective and co-management arrangements were found to weaken, both at the municipal management level and at staff meetings and for social participation. In the post-pandemic context, Primary Health Care is challenged to restore these arrangements and care for health workers exhausted by the pandemic.
