Is there an anatomic reason for the orthopaedic treatment of vertebral wedge fractures?

The authors present a prospective anatomical study of 41 vertebral crush fractures without posterior wall involvement in 31 patients who were treated functionally. The objective of the study was to measure the effect of this type of treatment on the outcome of vertebral deformity. For each vertebra the kyphosis angle and the regional post-traumatic kyphosis was measured at the end of treatment. The angle of loss was therefore calculated for each vertebra. The figures showed a slight increase in deformity independent of the initial degree of crushing. They conclude that from an anatomic viewpoint reduction and immobilisation according to the Böhler technique has no advantages over a functional treatment in the management of a thoracic or lumbar wedge fracture.
