•Non-genotoxic immunosuppressants (IS) have a threshold mechanism for causing cancer.•Exposure to IS below the effect threshold results in low risk for causing cancer.•Cyclosporine should be considered to have low risk for causing cancer ⩽17.5μg/m3.
