The primary immune deficiency disorders (PIDDs) are a group of chronic non-communicable diseases (NCDs) which manifest primarily with a wide spectrum of infections. The pattern of infections is dependent on the underlying immunological defect, and geographical and social determinants. Globally, there has been a decline in mortality caused by communicable, maternal, neonatal, and nutritional diseases, and a converse increase in mortality due to NCDs. However, the degree to which epidemiological transition from infectious diseases to chronic NCDs has occurred differs in the Middle East and North Africa (MENA), and sub-Saharan Africa (SSA). In 2010, the leading causes of mortality in MENA were ischemic heart disease, cerebrovascular disease, lower respiratory infections, and preterm birth complications, reflecting the shift towards NCDs in that region. By contrast, in SSA the leading causes of mortality remain communicable and nutritional diseases, primarily HIV/AIDS, lower respiratory infections, diarrheal diseases, malaria, and protein-energy malnutrition. The two regions have a combined population of 1.2 billion people, or 17.4% of the global population. However, only 7.7% of patients with PIDD reported in a recent global survey resided in MENA or SSA, suggesting that under-diagnosis and under-reporting are prevalent.
