Hairy cell leukemia (HCL) is a rare hematologic malignancy. Unusual locations of the disease are sometimes seen, but remain rare. We report a patient with HCL treated with interferon, who develops during her illness a mediastinal pleural mass, discovered incidentally after trauma. This mass makes us to discuss a cancerous origin, a trauma, an infection or a specific location of the disease. The diagnosis of specific localization was retained. Treatment with cladribine was effective to regress and to remove the lesion.
