Analysis of medication adherence-related notes from a service-oriented community pharmacy.

•Previous research shows pharmacists may not be regularly evaluating patient medication non-adherence in the refill process.•PDC calculations can alert pharmacists of potential medication nonadherence, and spur pharmacist-patient dialogue.•Many nonadherence alerts were found to be false positives or a situation to monitor.•Intentional reasons for nonadherence were rare, contrary to trends in the adherence literature.
