•High-priority interaction alerts were mostly present in five international EHRs.•Overriding high priority alerts was too easy.•Low-priority DDI alerts were not present in most EHRs.•There is a lack of standardization in DDI levels in EHRs in different countries.•Research on achieving the right balance between over- and under-alerting is required.
