•Panic attacks are common in smokers with a cancer diagnosis (21.9%).•Panic attacks are associated with decreased likelihood of smoking abstinence.•Early abstinence, but not later, were affected by panic attack status.•Panic attacks should be addressed as barriers to cessation in cancer patients.
