•To date, this study is the longest combined neurological, neuropsychological and radiological longitudinal follow-up of MS.•Cognitive impairment on a group level develops linearly in MS and is only partly related to physical disability.•Cognitive disabilities are mainly associated with lesion accumulation and corpus callosal atrophy.•Lesion fraction is an independent predictor of cognitive performance 8.5 years later.
