•Biomarkers are assessed to predict disease severity in febrile urinary tract infection.•MRproADM has a higher predictive value for a complicated course than PCT or CRP.•MRproADM and PCT were significantly higher in patients directly hospitalized.•Outpatients requiring subsequent hospitalization had higher MRproADM concentrations.•MRproADM guided triage may decrease hospitalization rate without compromising safety.
