•Alcoholic liver disease is the main indication for liver transplantation in France and one of the main indications worldwide.•De novo malignancies after liver transplantation for alcoholic liver disease is a frequent and life-threatening long-term complication.•The main risk factor for de novo malignancies after liver transplantation for alcoholic liver disease is tobacco smoking (both past and active).•Using mTOR inhibitor-based immunosuppressive regimen could reduce this risk for de novo malignancies after liver transplantation for alcoholic liver disease.
