•Gastroesophageal reflux disease (GERD) in CF patients poses a risk for Barrett's esophagus (BE)•Patients with CF had a three-fold increased odds of BE or neoplasia•The mean age of diagnosis of BE among CF patients was younger than reported in the non-CF population•Majority of BE cases had undergone solid organ transplant•Upper endoscopy should be considered in CF patients with chronic GERD, especially among transplant recipients
