•Application of the 2013 AHA/ASA Guidelines at Assaf Harofeh Medical Center in Israel resulted in expanded eligibility for TPA, a 64% increase in its use, and no adverse safety signals•Higher patient volumes were associated with a rise in home discharges (33% to 48%) in similarly affected patients with NIHSS scores >6.
