In rheumatology practice, the fibromyalgia syndrome (FMS) is a crucial differential diagnosis in patients with chronic nuchal discomfort. A number of inflammatory rheumatic diseases manifested in the area of the cervical spine must be differentiated from FMS. They include rheumatic polymyalgia, rheumatoid arthritis, axial spondyloarthritis and dermatomyositis/ polymyositis. On the other hand, chronic pain syndromes may also occur in association with inflammatory or degenerative diseases or as a posttraumatic condition.
