Clinical features and prognosis of Chinese sarcoidosis patients.

Purpose: To summarize the clinical features and prognosis of Chinese sarcoidosis patients. Method: Retrospectively collected sarcoidosis patients in Beijing Chaoyang Hospital from 2007 to 2015, summarize the characteristics, the basic population of clinical manifestation, imaging feature, laboratory examination results, the therapeutic regimen and prognosis. The prognostic factors were analyzed also. Result: A total of 229 sarcoidosis patients were collected, 188 of whom were pathologically confirmed. There were 74 males and 155 females, with a male to female ratio of 1:2.09. The peak age of male onset is 35~40 years old, and the peak age of female onset is 50~55 years. The main clinical manifestations were cough, shortness of breath, and fever. One hundred and fifty-four patientd of stage II and III received glucocorticoid treatment, 2 cases were treated with azathioprine. This study was carried out in 132 patients from 2 to 10 years of follow-up, 1 people died of multiple myeloma, 89 patients after prednisone therapy get ease and stability, 43 cases relapse after remission. Multivariate analysis found that higher initial serum angiotensin converting enzyme (sACE) level is an independent risk factor for recurrence of sarcoidosis (RR=1.56, p=0.032), while the oxygenation index, lung function, bronchoalveolar lavage fluid risk lymphocyte ratio and CD4+/CD8+ ratio were not factors of recurrence. Conclusions: Elevated serum angiotensin converting enzyme (ACE) is a risk factor for recurrence in sarcoidosis patients, that may suggesting that patients with high granulomatous burden at the time of diagnose are more likely to relapse.
