•False decrease of hs-cTnT occurred in hemolyzed samples transported by PTS.•Negative bias of hs-cTnT assay was related to the hemolysis degrees in PTS specimens.•Preanalytical errors of hs-cTnT assays in hemolyzed PTS samples (free Hb ≥50 mg/dl) should be considered in clinical practice.
