Osteonecrosis of the distal tibia after pilon fractures

•Distal tibial epiphyseal osteonecrosis can occur following pilon fractures.•Osteonecrosis diagnosed radiographically and intraoperatively.•Not associated with smoking, open fractures, diabetes, or age.•Associated with size of fracture fragments, comminution, and calcium phosphate bone substitute.
