Implementing a pulmonary function laboratory (PFL) quality assurance (QA) program across a large health system

Background: The ATS-ERS have established QA models as components of their PF technical standards (TS), however other than calibration many are not performed. We wanted to ensure high quality PF test results across the >70 facilities which comprize the Mayo Clinic (MC) health system. Objective: Implement a QA program for all lung function testing across the MC enterprise. Establish the individual components and QA metrics if not defined in the TSs. Methods: Basic training was defined as completion of a 2-day spirometry course for all personnel if not already specifically certified in PF testing. Ensure mechanical and biological models were being performed, set the target ranges if not defined by the TS, (Table 1). We developed a method to document the processes. A Sharepoint (figure 2) was created to facilitate communication, provide resources, house forms for documentation including spreadsheets with target limits and CVs. Conclusion: Standardized training and reporting tools, setting the target criteria for QA models, and a single reporting location have been essential elements in implementing the MC PFL QA program.
