•Autoimmune encephalitis associated with anti-Glutamic Acid Decarboxylase 65 (GAD65) may be not rare.•Refractory status epilepticus and cognitive impairment were common initial presentations of anti-GAD65 encephalitis.•The abnormal signals of MRI may be obvious in bilateral frontal, temporal lobe and hippocampus.•The production of anti-GAD65 may have a certain latency period, and was usually negative at the onset stage.
