CHANGES IN THE EXPRESSIONS OF SERUM HOMOCYSTEINE, FOLIC ACID AND VITAMIN B12 IN DIABETIC NEUROGENIC BLADDER AND THE RELATIONSHIP WITH BLADDER FUNCTION

Objective: To investigate changes in the expressions of serum homocysteine (Hcy), folic acid (FA) and vitamin B12 (VitB12) in patients with diabetic neurogenic bladder (DNB), and the relationship with bladder function. Methods: Chosen for the study were 184 patients with type 2 diabetes mellitus (T2DM) admitted to Second affiliated hospital of Nanjing Medical University for treatment from January 2018 to August 2019, of whom 72 were DNB patients (the DNB group) and 112 were non-DNB patients (the non-DNB group). Serum Hcy, FA and VitB12 were detected in patients of both groups, and the correlation between serum Hey, FA and VitB12 and bladder function of the DNB patients was analysed in the study. Results: The levels of serum Hey (22.71 +/- 6.84 mu mol/L), fasting plasma glucose (FPG) (8.15 +/- 2.07 mmol/L) and triglyceride (TG) (2.08 +/- 056 mmol/L) in the patients of the DNB group were significantly higher than those of the non-DNB group (10.06 +/- 4.41 ,umoll L, 7 .52 +/- 1 .74 mmol/L and 1.73 +/- 0.64 mmol/L) (P<0.05). The levels of folic acid (FA) (13.62 +/- 4.05 mu g/L), VitB12 (268.93 +/- 107.62 mu g/L) and fasting insulin (Fins) (0.56 +/- 0.24 mmol/L) in the patients of the DNB group were significantly lower than those of the non-DNB group (20.11 +/- 5.84 mu g/L, 358.22 +/- 124.68 mu g/L and 0.77 +/- 032 mU/L) (P<0.05). The bladder functional urethral length (FUL) and maximum urethral pressure (PMU) in the patients of the DNB group were significantly higher than those of the non-DNB group (P<0.05). The level of bladder compliance (BC)for the patients of the DNB group was obviously lower than that of the patients in the non-DNB group (P<0.05). In the patients of the DNB group, the levels of FA and VitB12 were positively correlated with BC (P<0.05), the level of Hcy was negatively correlated with the level of BC, PMU was negatively correlated with FA and VitB12, and PMU was positively correlated with Hey (P<0.05). Conclusion: The expression levels of Hcy, FA and VitB12 changed considerably in the patients of the DNB group, and they were closely correlated with patients' bladder function.
