Functional outcome of gastrocnemius advancement flap augmented with short flexor hallucis longus tendon transfer in chronic Achilles tear

•VY plasty or fascial turndown flap of gastrocnemius muscle may fill the defect but not sufficient enough to provide strength in chronic Achilles tear.•The gastrocnemius advancement flap with short flexor hallucis longus tendon transfer had excellent clinical outcome in chronic Achilles tear.•Short flexor hallucis longus tendon transfer in Achilles tear has advantages of same phasic action, ease of harvesting and excellent tensile strength.•It restored the push off strength, and there was minimal morbidity.•All patients resumed their preinjury activities and there was no re-rupture.
