•Secondary mitral regurgitation is a common finding in patients with chronic heart failure with reduced left ventricular ejection fraction.•Atrial fibrillation frequently occurs simultaneously with the onset of significant secondary mitral regurgitation, and further worsens the prognosis of patients.•In patients undergoing percutaneous mitral valve repair with MitraClip, preoperative atrial fibrillation is associated with worst 5-year outcome in terms of overall death and cardiac death.•No difference in re-hospitalization for heart failure was observed among patients with or without atrial fibrillation.
