•Dual acting therapeutic proteins may be a better strategy than drug combinations for treatment of chronic retinal diseases.•Several targets to treat neovascularisation and inflammation have been identified.•A dual acting molecule to increase the duration of action in the vitreous has potential for treatment of chronic retinal diseases.•The key challenge in developing intraocular medicines is ensuring there is minimal ocular toxicity.•Maintaining protein stability and avoiding protein aggregation and misfolding are necessary.
