“The perfect breast”: Measuring cosmetic outcomes after breast-conserving therapy

•A standardised, easy to use tool to measure breast cosmesis after breast-conserving therapy is lacking.•We report a user-friendly subscale based semi-objective scoring system (Total Cosmetic Score).•It will also identify the cause for cosmetic deficiency, to guide corrective and preventative measures.
