•Appropriate LEV dose for seizure control during awake craniotomy is unclear.•LEV showed higher correlation between the serum concentration and adjusted dose.•The serum LEV concentration was relatively low in patients with intraoperative seizures.•Monitoring the serum LEV concentration could be useful in awake craniotomy.
