Complete mesocolic excision (CME) is widely accepted as the optimal procedure for colon cancer resection [1]. However, the safety and feasibility of performing CME laparoscopically for locally advanced T4b colon cancer is controversial because it entails technically demanding surgery, including en bloc resection. [2]. Additionally, performing extensive lymph node dissection in the lateral pelvic area is challenging because of poor visibility and proximity to the iliac vessels, obturator nerve and various muscles.
