•A retrospective study on older patients with non-small-cell lung cancer was conducted.•Clinical impact of pembrolizumab plus chemotherapy was studied in these patients.•Pembrolizumab combined with platinum and pemetrexed leads to poor clinical outcomes.•Poor performance status and low albumin level were prognostic factors.
