•Opisthorchiasis and clonorchiasis are major public health problems affected over 15 million in the GMS.•Opisthorchis viverrini is endemic in Lower Mekong Basin while Clonorchis sinensis is endemic in Guangxi and northern Vietnam.•Snail intermediate hosts and geographical barrier are key determinants in distribution difference.•Emerging human liver fluke infections occur in Cambodia, Myanmar, and Guangxi over the past 20 years.•Systematic exploring of the disease burdens and integrated control using One Health approach are warranted.
