•RCT to investigate the effectiveness of a novel intervention to reduce ADRs following hospitalisation.•ADR risk-assessment and plan is guided by the PADR-EC score.•Information to reduce the risk of ADRs is communicated to patients, clinicians, general practitioners and pharmacists.•Participants will be followed for up to 24 months post-discharge to identify ADRs and ADEs.
