Diagnosis And Treatment Of Primary Malignant Lymphoma Of Bone

Objective :To explore the clinical features as well as the treatment outcome of PLB. Methods: The clinical data of 11 cases of PLB admitted and treated from April 1995 to October 2003 in our hospital were analyzed retrospectively. The diagnose was confirmed histological.11 patients with the primary site of tumors(ilium in 3, thoracic spine in 2,lumbar spine in 2,cranium in1,ulna in 1,femur in 1) were reviewed. The most often presenting symptom was pain, One case of lumbar lymphoma had dysfunction of nerve or spinal cord.X-ray showed osteolysis and erosion.Most of histological types were diffuse B-cell lymphoma. Local radiotherapy combined with chemotherapy was conducted,One patient with lumbar Pathological fracture received percutaneous vertebroplasty(PVP),and one received artificial hip joint replacement. Results: The follow-up range 1 to 8 years,average was 2.5 years.The 1-year ,3-years and 5-years survival rates were 100% (11/11),66.7% (2/3) and 50.0% (1/2), respectively. One case of lumbar lymphoma died of mediastinum metastases.The survival rates decreased with the progression of the clinical stage. Conclusions : PLB is a rarely occurring malignant bone tumor. Combination therapy including radiotherapy and chemotherapy are main treatment for PLB,surgery is an adjuvant measure to treat or prevent pathologic fractures.
