Intramedullary Epidermoid Cyst: A Case Report

IntroductionEpidermoid cysts are benign tumors with generally congenital origin. In the spinal localization intradural extra-medullary is the common localisation, rarely intramedullary. We reported a new case of intramedullary epidermoid cyst with dorsal localization and we discuss the ethiopathological, epidemiological, clinical, radiological characteristics and the neurosurgery treatment.Case reportA 16 years-old patient was admitted for spastic and progressive paraparesia and incontinence of urine 18 months ago without sensitive troubles. Medullary MRI showed fusiform intramedullary lesion at the level T12. The lesion was hypointense on T1, heterogenous on T2 with discreet enhancement on T1 weighted image after Gadolinium injection. A posterior mid line myelotomy was performed followed by a total excision of the lesion. There were white flaky fragments. Histopathological exam concluded of epidermoid cyst. Post operatory outcome was good (follow up of six months).ConclusionEpidermoid cysts are rare benign tumors. Intra-medullary localization is incommon. The diagnosis was suggested by the resonance magnetic imaging. The treatment was neurosurgical. The prognosis was generaly good.
