•We survey over 4000 people who avoided transit during COVID-19.•We asked respondents if avoiding transit meant deferring healthcare.•We asked if avoiding transit made reaching healthcare and prescriptions difficult.•Non-white, disabled, and low-income riders were more likely to defer care.•Vehicle access reduced the likelihood of a negative outcome across all models.
