Value of the capsular enhancement sign on dynamic contrast-enhanced prostate multiparametric MRI for the detection of extracapsular extension

•We propose a novel imaging parameter on DCE, capsular enhancement sign (CES).•The presence of CES on DCE has high specificity and PPV for prediction of ECE.•Lesions with CES show a higher tumour grade, perineural and lymphovascular invasion.•The presence of CES should enable greater reader confidence for calling ECE.
