•Cell of origin, etiology and pathogenesis of juvenile xanthogranuloma (JXG) are not fully understood.•A combination of CD11c, CD4, CD1a and either CD163 or CD68 stains is recommended when evaluating for differential diagnosis.•Molecular basis of JXG is yet to be understood.
