•Epidural-related maternal fever (ERMF) increases risk of obstetric interventions.•ERMF may be linked to anti-pyrogenic interleukin-1 receptor antagonist (IL-1ra).•EPIFEVER-2 is a prospective study of ERMF in 637 women in labour.•Mendelian randomisation analysis will examine IL-1ra polymorphisms and ERMF.
