•The levels of serum CTRP4 were lower in T2DM patients with carotid atherosclerosis (CAS) compared to those without CAS.•Serum CTRP4 might be an independent influence factor for CAS in T2DM.•Serum CTRP4 levels were negatively related to the risk of CAS in T2DM.
