CoTePrAgmatique (CTPA) hotline for advice on antibiotic prescription to general practitioners: results of 2 years activity

Context. In France, Antibiotic (ATB) consumption in general practice remains one of the highest in Europe despite a decrease between 2000 and 2008 and 3 "antibiotic plans". CoTePrAgmatique (CTPA) hotline was dedicated to general practitioners (GPs) in Ile de France, from April 2013 to March 2015 thanks ARS's funding. The aim of the line was to improve the relevance of ATB prescriptions by answering of GPs questions in care situation. Objectives. Describe working and results of CTPA hotline. Method. Retrospective and Descriptive cross-sectional study from collection sheets completed by effectors between April 2013 and March 2015. Number of calls, pathology's class and advice given were studying in describe analysis. Univariate and multivariate multilevel analyzes factors associated with ATB non-prescription advice. Results. The line recorded 517 calls at the end of March 2015, 36% was made for the urinary tract and 23% for upper and lower respiratory infections (IRH, IRB). Non-prescription of ATB advice was given in 39.3% of the cases, the factors associated with this advice were calls concerning IRH (odds ratio (OR) = 2.62; IC95 = 1.31 - 5.27, other infections (OR = 3.30; IC95 = 1.44 - 7.55), and the absence of ATB in progress (OR = 1.81; 1C95=1.18-2.79). Conclusion. Despite a modest number of calls, the line achieved its objectives. The development of a global project integrating phone counseling and other existing tools, with institutional communication seems to be a credible perspective to reduce the inappropriate ATB prescription.
