•LBCL-IRF4 can exhibit a wide variety of morphology.•A LBCL with GCB immunophenotype but strong and uniform MUM1 should raise the suspicion for LBCL-IRF4.•LBCL-IRF4 should be considered in young patients with lymphomas in the head and neck region.
