•This study presents data on CAD severity in patients with acute MI and high Lp(a).•Patients with high Lp(a) had poorer survival during hospitalization for acute MI.•Very high Lp(a) levels (> 100 mg/dL) are associated with a severe CAD burden.•A call to action is needed for Lp(a) screening in all patients with acute MI.
