Hepatopulmonary syndrome: Unique radiological association with portal vein thrombosis and porto-systemic shunting

•Patients with hepatopulmonary syndrome have shared radiological findings of portal vein thrombosis and splanchnic dilation with a porto-systemic shunt.•Portal vein thrombosis with splanchnic vein dilatation can lead to intrapulmonary vascular dilation and is a potential cause of hepatopulmonary syndrome .•Hepatopulmonary syndrome improves with liver transplantation.
