Left atrial (LA) pressure overload is an important mediator of atrial remodeling and stretch-induced atrial fibrillation (AF). Mechanoelectrical alterations promote atrial myocyte hypertrophy, fibrosis, and potassium gene expression with resultant proarrhythmic spatiotemporal changes in atrial action potential. Increased LA pressure is known to predict incidence and recurrence of AF. An Inter Atrial Shunt Device (IASD) is an experimental treatment for heart failure with preserved ejection fraction (HFpEF), used to reduce LA pressure and improve HF symptoms.
