•Predictive and prognostic factors in anal cancer are limited.Bio-humoral prognostic factors are easy to assess and use.•The Hemo-Eosinophils Inflammation (HEI) Index has been recently proposed for anal cancer.•HEI was confirmed to be a prognosticator in anal cancer patients.•Model discrimination in the external validation cohort was acceptable.
