•XFEM is here used to follow crack growth and possible failure in the mandibular reconstruction plate under fatigue loads.•The interaction between bone remodeling and fatigue damage is investigated.•The study focusses on the importance of bone remodeling on the fatigue life of fixation plates.•The numerical analyses are correlated with specific clinical follow-up.
