•Little is known about metabolic syndrome (MetS) and clot lysis in Africans.•Association of PAI-1act and CLT with MetS over a 10-year period was determined.•Fibrinolysis was inhibited in MetS; throughout the 10-year period in Africans.•Obesity and lipids were the MetS criteria most strongly associated with CLT and PAI-1act.
