•Helios gene expression decreased in SPMS patients.•The percentage of Helios+ Tregs was low in the peripheral blood of SPMS patients.•One of the immunological mechanisms involved in SPMS may be a deficiency in Tregs.•The percentage of CD4+CD25+ T lymphocytes did not show a significant difference.
