•The best values of IrOA are obtained by UPs on H&E slides, with OCT4/CD34 not improving the IrOA in all groups.•CD modify the LVI in a significant number of cases, but the effect on the pT re-staging is less relevant.•The main sources of disagreement among UPs are artefactual clefts and histiocytes within the vessels on H&E slides.•The main sources of disagreement among UPs are artefactual clefts and floaters on OCT4/CD34 slides.
