•Vessel injury impacts in-stent restenosis but its assessment in vivo is challenging•Several injury scores have been established for postmortem injury assessment•We suggest an in vivo injury score to predict restenosis after stent implantation.•OCT-based assessment of this injury score might enable patient-individualized PCI.
